What incidents must be reported to the MOL in Ontario?

Under OHSA s.51-53: critical injuries or fatalities (immediately by phone, then written within 48 hours), work refusals, and occupational illnesses. A critical injury is defined in O.Reg. 834 and includes: loss of consciousness, fracture, amputation, loss of sight, and burns requiring medical treatment.

What is the difference between root cause and contributing factors?

A root cause is the fundamental system failure that, if corrected, would have prevented the incident. Contributing factors are conditions or actions that increased the likelihood or severity but were not the primary cause. A thorough investigation identifies both.
